Cancer-related fatigue

Cancer-related fatigue (CRF) is a distressing, persistent sense of physical, emotional or cognitive tiredness not proportional to recent activity and not relieved by rest. It affects 70-100% of patients during active cancer treatment and persists in 30-40% of long-term survivors. UK NHS survivorship resources, Macmillan, ESMO, NCCN and selected NICE cancer guidance emphasise multimodal management: exercise (the single highest-evidence intervention), psychological support, sleep hygiene, treatment of contributing factors (anaemia, thyroid, depression) and judicious pharmacotherapy.

Definition and epidemiology

  • NCCN definition: "a distressing, persistent, subjective sense of physical, emotional, and/or cognitive tiredness or exhaustion related to cancer or cancer treatment that is not proportional to recent activity and interferes with usual functioning."
  • Prevalence: 70-100% during treatment; 30-40% in 1-5 year survivors; long-term persistence in subsets.
  • Most common cancer-related symptom; under-reported.
  • Quality-of-life impact significant; affects function, sleep, mood, relationships, work.

Contributing factors

  • Treatment-related:
    • Chemotherapy.
    • Radiotherapy.
    • Immunotherapy (ICI).
    • BRAF / MEK inhibitors.
    • Targeted therapies.
    • Surgery (especially major reconstruction).
  • Tumour-related:
    • Pro-inflammatory cytokine release (IL-6, TNF-ฮฑ, IL-1ฮฒ).
    • Tumour burden, cachexia.
    • Tumour-driven metabolic abnormalities.
  • Concurrent factors:
    • Anaemia (chemotherapy / chronic disease).
    • Hypothyroidism (ICI-related, primary).
    • Hypogonadism / adrenal insufficiency (especially after ICI).
    • Electrolyte disturbance, dehydration.
    • Pain.
    • Sleep disturbance, insomnia.
    • Depression, anxiety.
    • Polypharmacy, particularly opioids, anticonvulsants, antihistamines.
    • Deconditioning, inactivity.

Assessment

  • NCCN screening: brief 0-10 NRS at each clinic visit ("Rate your fatigue in the past 7 days").
  • โ‰ฅ4 โ†’ comprehensive evaluation.
  • Tools: FACIT-Fatigue, Brief Fatigue Inventory (BFI), MFI-20, Cancer Fatigue Scale.
  • Investigations for treatable contributors:
    • FBC, ferritin, B12, folate.
    • U&E, calcium, magnesium.
    • TFT, cortisol, testosterone, free T4 (low threshold post-ICI).
    • Vitamin D, glucose / HbA1c.
    • LFT.
    • Sleep history, mood screen (HADS, PHQ-9).

Management

  • Treat contributing factors: anaemia (iron / EPO if indicated), hypothyroidism, hypogonadism, depression, sleep, pain.
  • Non-pharmacological (highest evidence):
    • Exercise: 150 min / week moderate aerobic + 2ร— weekly resistance. Strong evidence in Cochrane review.
    • Tailored physiotherapy / cancer rehabilitation programme.
    • Cognitive behavioural therapy (CBT) for cancer fatigue.
    • Mindfulness-based stress reduction.
    • Energy conservation techniques.
    • Sleep hygiene.
    • Yoga, tai chi.
  • Pharmacological (limited evidence; selected cases):
    • Methylphenidate 5-30 mg / day โ€” modest benefit in advanced disease.
    • Modafinil โ€” limited evidence; not NICE-recommended for CRF.
    • Antidepressants for concurrent depression.
    • Corticosteroids (short-term) in end-of-life setting.
  • Survivorship: integrated rehabilitation programme; Macmillan resources; cancer-specific gym schemes.
  • Counsel: CRF is real and treatable; many patients fear it as "cancer return" โ€” reassurance plus active management.
